How do we know if the claims went through? Are you getting paid by your insurance company? Do you have all the ICD-10 codes you need?

ICD-10 is the 10th revision of the International Statistical Classification of Diseases and Related Health Problems (ICD), a medical classification list by the World Health Organization (WHO). It contains codes for diseases, signs and symptoms, abnormal findings, complaints, social circumstances, and external causes of injury or diseases.

The code set allows more than 14,400 different codes and permits the tracking of many new diagnoses. The codes can be expanded to over 16,000 codes by using optional sub-classifications. The detail reported by ICD can be further increased, with a simplified multi-axial approach, by using codes meant to be reported in a separate data field.

The International version of ICD should not be confused with national Clinical Modifications (CM) of ICD that frequently include much more detail, and sometimes have separate sections for procedures. The US ICD-10 Clinical Modification (ICD-10-CM), for instance, has some 68,000 codes. The US also has the ICD-10 Procedure Coding System (ICD-10 PCS), a coding system that contains 76,000 codes not used by other countries.

The GEMs are a tool that you can use to convert data from ICD-9-CM to ICD-10-CM and ICD-10-PCS and vice versa. Medisoft 19, Medisoft 20, Lytec 2014 and Lytec 2015 mapping utility utilized the GEM tool. The GEMs are also known as crosswalks as they provide important information linking codes of one system with codes in the other system. The GEMs are a comprehensive translation dictionary that can be used to accurately and effectively translate any ICD-9-CM-based data, including data for:
™ Tracking quality, Recording morbidity/mortality, Calculating reimbursement, or
™ Converting any ICD-9-CM-based application to ICD-10.

Mapping from ICD-10-CM and ICD-10-PCS codes back to ICD-9 codes is known as backward mapping. Mapping from ICD-9 codes to ICD-10 and ICD-10 codes is known as forward mapping. The GEMs are complete in their description of all the mapping possibilities as well as when there are new concepts in ICD-10 that are not found in ICD-9. All ICD-9 codes and all ICD-10 codes are included in the collective GEMs:
™ All ICD-10 codes are in the ICD-10-CM to ICD-9 GEMs.
™ All ICD-9 diagnosis codes are in the ICD-9 to ICD-10 GEMs.
™All ICD-10 codes are in the ICD-10-PCS to ICD-9 GEMs.
™ All ICD-9-CM procedure codes are in the ICD-9-CM to ICD-10-PCS GEMs.
